LEVEL VI: BIO MATERIALS & PRINCIPLES OF SITE DEVELOPMENT

COURSE OBJECTIVE

With hundred of products to choose from, clinicians are often in a dilemma as to which product to use in a particular clinical scenario. Every clinician asks these question as to.  – “Which product will work better in a given situation?  How to correctly use it ? and When to use it” ? – Get all these answers from world renowned scientist, surgeon and researcher with no bias and prejudice for products.

Understand the biology of bone regeneration & bone graft healing, the biologic inter-dependency of soft and hard tissues and the anatomic basis for intra-oral bone augmentation at anatomically compromised implant sites. This course is designed to give clinicians the confidence to perform everyday bone grafting procedures.  Choosing an appropriate graft material, membranes and other related products with a clear understanding of the biological basis of each product as it pertains to implant dentistry. This course will clearly outline most commonly used products with advantages, disadvantages, and applications to help clinician achieve predictable results. Each participant will be able to diagnose cases for appropriate usage of biologic products and to recognize when not to use a product.

Hands on portion of the course will bring things into perspective for better handling of the products.

COURSE OUTLINE
DAY 1

SESSION 1

Biology and Principles of Intra-oral Bone Grafting & Regeneration

  • Principles and scientific concepts of tissue engineering
  • Factors that favor successful results for Intra-oral bone grafting
  • Factors related to graft shrinkage and failure
  • Ideal properties of bone graft substitutes
  • Indications for and selection of autogenous bone grafts and graft substitutes
  • Indications for and selection of Barrier membranes
  • Indications for and selection of blood component therapy ( PRP, PRF)

Fundamental Material Science & Instrumentation for Intra-oral bone grafting

SESSION 2

  • Applications of tissue engineering for bone augmentation at complex implant sites
  • Patient Evaluation and Treatment Planning for Intra-oral Bone Augmentation
  • Patient and Local Site Evaluation & Risk Assessment
  • Determining the Degree of difficulty for bone grafting: ( Patient & local site factors)
  • Role of Cone Beam CT in reducing donor and recipient site morbidity for intra-oral bone grafting

Soft Tissue Considerations for Successful Intra-oral Bone Augmentation

  • Principles of soft tissue flap management for bone augmentation procedures
  • Determining the need for soft tissue augmentation and when to perfor
  •  Step-by-step procedures for soft tissue incisions, flap designs and maneuvers that ensure passive closure

  • Considerations for interim provisional restorations & Peri-operative patient management

Staged Reconstruction with Block Grafts & Guided Bone Regeneration Procedures:

  • Diagnosing and Treatment Planning (Clinical Evaluation)
  • Recipient site requirements and preparation for successful bone augmentation
  • Step-by-step procedures for autogenous block grafts for horizontal & vertical Augmentations
  • Step-by-step procedures for allograft block with GBR for horizontal Ridge Augmentations
  • Considerations for interim provisional restorations & Peri-operative patient management

Bone Spreading and Ridge splitting procedures for Alveolar Ridge Expansion

SESSION 2

  • Diagnosing and Treatment Planning (CBCT & Clinical Evaluation)
  • Indications, instrumentation, limitations and step-by-step procedures

3D Biologic Techniques for Significant Horizontal and Vertical Ridge Augmentation

  • 3D Alveolar Ridge Reconstruction Procedures Using Autogenous Bone
    Corticocancellous Block Grafts
    Cortical block bone grafts cribs with trabecular bone graft
    Bone transport via osteotomy and Inter-positional graft
    Buccal plate repositioning-stabilization procedure with Inter-positional graft

Synergistic Tissue Engineering Approach to Reconstruction of Complex Horizontal Ridge Defects

  • Diagnosing and Treatment Planning.
  • Indications, instrumentation, limitations and step-by-step procedures

Management of Intraoral Bone Grafting Complications

HANDS-ON EXERCISES will include treatment planning modalities, Advanced GBR, block grafting, ridge expansion, and augmenting rebreed sites.
HANDS ON MODULES

Treatment Planning Immediate Implants will start with establishing diagnosis and treatment plans on models for Advanced GBR, block grafting, ridge expansion, and augmenting rebreed sites. Different techniques and modalities will be practiced.

Low speed PRF Protocols will include, detailed hands on protocol for  PRF and iPRF. Fabrication of steaky bone using PRF and iPRF. Manipulation and usage of PRF products augmentation procedures.

Autogenous Bone Blocks and Flap Management will include incisions and flap designs on the pig jaw ; harvesting blocks from ramps, chin areas; Preparing a recipient site; angiogenesis and space management using different techniques.manipulation of surgical flaps;  obtaining of tension free closure ;  elongating flaps for primary closure ; Ridge expansion and splits using piezo surgery and discs ; harvesting bone cylinders ; tacking membranes using screws for ridge augmentation and 3 step suturing techniques.

Limited enrollment ensures one on one interaction between the instructor and participants to enhance the learning experience.
